To book a job, fill in the request form and I will give you a costing, if approved, you will need to fill in and return this hire form. This form is from you to me. You can download the form by clicking on the Word icon, 
an (example only) is shown on this page. You can attach and send the form via

You understand that the work performed is regarded as “work for hire.” [Company Name]shall own all rights to the work in its name or otherwise.
You warrant that the work will be an original work that has not been in the public domain or previously created, and that the work will be free of any unauthorised extractions from other sources. You further understand that [Company Name] has the privilege of referring to you as a vendor in sales and marketing presentations and materials.
